Permission Slips

To get a permission slip for an upcoming event, you must download the PDF permission slip, print it to your own printer or other local printer, have your parent or guardian sign it, have it notarized, and hand it in at the next service, send it in the mail or fax it to us at 623.842.5081, attention Minister Chris Juhl.  Please remember all permission slips must be notarized.
